I understand that sometimes things come up in the way of plans, but this is a consistent problem and I'm starting to feel like you don't value my time. Please stop making plans and cancelling suddenly the day of, I'd like at least 24 hour notice if possible.

Even if you don't get what you want its still good to communicate assertively as people will more often keep in mind your feelings on the subject, and its good to keep it up as a habit.
