Monitor and Plan
Maybe change the way you are writing the notes. Maybe review the lesson the night before. Or look at past homework or tests before a test or a big homework assignment. But i think it all starts in the classroom if you aren't paying attention you won't do well. But if you are paying attention and you still aren't doing well then it could well be you need to change the way you are noting and studying.
