I know you may have a lot going on and that is fine, I would like to know if something is bothering you and if that is the reason you keep canceling our plans, if there is anything I can do to help you let me know and I am always here if you need me.

You should be assertive because you know you will always be honest with the person while communicating with out hurting there feelings. another reason you should be assertive is because you can be direct with the person reducing conflict in the long run and enhancing personal relationships.
