Every time we try to make plan together you cancel last minute. Is there something I have done to upset you? I would like to hangout with you but if you continue to cancel on me I don't want to hangout with you.

Being honest and polite about something can get your point across just as much as yelling and being disrespectful. Sometimes being aggressive towards is the only way for them to understand. For other people being nice about it can get your point across without being rude.
