I don't like when you cancel on me last minute, it makes me feel disregarded and disliked by you. Next time we plan something, I would like for us to plan an exact date and time so that we can plan around that.

You should choose to communicate assertively because it makes you seem more confident, and it can make the other person/people included in the conversation think about things they do because they do them and/or repeat their mistakes.
