Whether you don't do it on purpose or not, when we make plans to hang out and cancel on me last minute, that makes me think you don't want to and are messing with me and just wasting that time I use to wait on you. I think we could figure out a solution. I would appreciate if we could make one plan work and go on from there.

You should still communicate assertively so you can get your point across and make sure you are fair in the discussion that you are having.
